To participate, an athlete must complete the following forms: health history questionnaire (complete PRIOR to the beginning of each season) risk of injury statement (complete PRIOR to the beginning of each season) training regulations (complete PRIOR to the beginning of each season) physical (within the last 12 months) The school physician must review and sign all physical forms. emergency medical release
