Health History Questionnaire
Prior to EACH sport season, a health history questionnaire (available in the Health Office) has to be completed and returned to the Health Office, and an updated physical within 12 months is required. Athletes having a physical with their own provider must return the physical form to our Health Office by March 2. Our school doctor must sign off all outside physical forms.
Physicals will be given at our Health Office on 2/23/10 and 3/2/10. You can sign up for a school physical in the Health Office.
